Do I need an appointment?

Yes, all clients must schedule an appointment. Appointment are scheculded Monday-Friday between the times of 9am-2pm.

Am I covered by insurance?

If you are covered by State funded medicaid we are able to accept that as a form of payment.

Do I need insurance?

Yes, Long term care insurance. We are also able to take private pay clients through the form of payments such as debt and cards, checks and cash.

Do I need a medical history?

Yes, all clients must provide a medical history record.

What information should I bring?

Driver license or state ID and a copy of your social security card.
